Disability Support Services: Access

GP Information

The Disability Support Services Access team provides support for Aboriginal and Torres Strait Islander people and their families living with a disability between the ages of 0-65 to navigate the NDIS application process and obtain appropriate disability funding. 

Each Access journey is tailored to the individual needs of the client. The team will work closely with the client’s clinical team to gather supportive evidence for the NDIS Application to place them in the best position to obtain funding.

Locations

Access services the South East Queensland region.

Who to refer

Aboriginal and Torres Strait Islander people, aged 0-65 years who have:

  • a disability
  • chronic illness, mental health, or circumstances the impact the ability to live independently in the community.
