Disability Support Services: Direct Support
GP information

Disability Support Services delivers direct support and care by for Aboriginal and Torres Strait Islander people. 

Our team of NDIS support workers provide services including:

  • in home support – meal preparation, cleaning and household chores, and personal care such as showering and dressing
  • community access – assistance with shopping, transport, health appointments, attending social or community events.

Locations

  • In home support services and community access supports can be provided for Mob living in the Moreton Bay / Brisbane North region.
  • For Mob living in other regions, Disability Support Services provides connection to other service providers.

Who to refer

Aboriginal and Torres Strait Islander people who have:

  • a disability
  • chronic illness, mental health, or circumstances the impact the ability to live independently in the community.

  • Create referral letter from template > Select from one of the following referrals:
  • Disability Access and QCSS > Named person as “IUIH Referral – NDIS Access and QCSS.”
