GP information

Disability Support Services provides support coordination to Aboriginal and Torres Strait Islander people.

Our team of support coordinators and social workers provide a range of services including:

support to understand plans and connections to the services and programs identified as a need
implementation of NDIS funding
advocacy – an unfunded program for Mob with an existing NDIS plan, but with no support coordination funding.

Support Coordination services the Southeast Queensland region from the Gold Coast, to Caboolture, to Ipswich.

Aboriginal and Torres Strait Islander people who have:

a disability, and
a NDIS plan, and
support coordination funding in their NDIS plan, or
no support coordination funding allocated, require assistance for support in engaging and managing services under their plan

Create letter
Create referral letter from template > Select from one of the following referrals:
- Disability Support Coordination > Named person as “IUIH Referral – Support Coordination.”
