Hospital in the Home (HiTH) is a joint initiative of the Institute for Urban Indigenous Health, Brisbane North PHN and Metro-North Hospitals to support Aboriginal and Torres Strait Islander people living in SEQ by providing the same high-quality care received in hospital, in the patients home.
Through the HiTH program, accredited General Practitioners and Nurses will care for Mob, increasing the number of Aboriginal and Torres Strait Islander people accessing propa medical care.
This “at home” medical support will also reduce the number of Mob discharging from hospital (against medical advice), due to the comfortable and culturally appropriate setting for the patient.

- Yarn to your Health Worker/Nurse/GP at any Moreton ATSICHS Clinic.
- Ask your doctor (while at the hospital) if you can be cared for by HITH.
