What is HiTH?

Hospital in the Home (HiTH) is a joint initiative of the Institute for Urban Indigenous Health, Brisbane North PHN and Metro-North Hospitals to support Aboriginal and Torres Strait Islander people living in SEQ by providing the same high-quality care received in hospital, in the patients home. 

Through the HiTH program, accredited General Practitioners and Nurses will care for Mob, increasing the number of Aboriginal and Torres Strait Islander people accessing propa medical care.

This “at home” medical support will also reduce the number of Mob discharging from hospital (against medical advice), due to the comfortable and culturally appropriate setting for the patient.

Benefits of HITH: 

1.  You may need to be assessed at the hospital first, but this pathway means you will be seen at a Rapid Access Clinic rather than waiting for a long time in Emergency. 
2. Your care is delivered by specific IUIH GPs, MobLink Care Coordinators and Nurses / other Allied Health support from the North Lakes team. 
3. Relax and heal in the comfort of your own home while accessing propa care.
4. No more worrying about who you’re sleeping next to, or people coming in and out all throughout the day and night.
5. You can have family or friends visit when you choose and not be restricted to certain hours.
6. HITH has better outcomes for Mob; being comfortable and surrounded by our Mob aids us in the healing process.

How do I access HITH? 

1. HiTH is available to Mob who live on the north side of Brisbane. From Kilcoy and Caboolture in the North, all the way to the north side of the Brisbane River. 
2. To access the service, you must need acute care and you need to be referred to the program by a Doctor. 
3. Be sure this is right service for you:

      - Yarn to your Health Worker/Nurse/GP at any Moreton ATSICHS Clinic.

      - Ask your doctor (while at the hospital) if you can be cared for by HITH.
