This is a culturally responsive and holistic family centered service for Mob to support their goals and wishes and optimise quality of life and comfort.
Our team walks alongside you, and your family, to access services, manage your symptoms and empower you to make deadly choices about your wellbeing and care now, and as your needs change.
Who are we?
Our team of care coordinators, nurses and general practitioners will assist you to access services best suited to your needs and wishes.
This may be mental health support, at home care, family support workers, physiotherapists, a dietician and more. We will work with you, your family and care team to achieve the best outcomes for you.
Our Services
Everyone’s journey is different. The Palliative and End-of-Life Care team will cater to you and your family’s individual needs and wishes.
We will be with you throughout your journey and provide support including:
- accessing appropriate clinical care
- advice and advocacy
- diagnosis support
- cultural, social and emotional support, for your overall wellbeing
- sorry business.
- outreach support – wherever you’re most comfortable
