Mob Link: Supporting Our Mob with Connected, Culturally Safe Care
Mob Link is a vital service supporting Aboriginal and Torres Strait Islander people across South East Queensland by helping individuals and families access the health, social, and community services they need. Backed by IUIH and delivered by a team of deadly nurses, GPs, intake officers and social workers, Mob Link walks alongside our mob every step of the way, making the health system easier to navigate and ensuring no one is left behind.
Available seven days a week, Mob Link provides culturally safe assistance over the phone, in hospital, in clinic, or even at home. Whether you need a yarn with a health worker, help organising an appointment, or support coordinating care, Mob Link is here to help.
Telehealth and On‑the‑Phone Health Advice
Mob Link offers convenient health support for when you can’t get in to see your regular GP. Through phone or telehealth appointments, the team can help with:
- Advice for managing coughs, colds, flu and COVID‑19
- Antiviral medications when required
- Early pregnancy support
- Mental health support
- General health advice
- Regular prescriptions (excluding restricted medicines such as opioids or benzodiazepines)
This service ensures Mob can still access timely care – even after hours, on public holidays, or from remote locations.
Connection to Health & Social Services
Navigating the health system can be tough, especially when juggling multiple needs. Mob Link makes it easier by connecting people with a wide range of services, including:
- Allied health: physiotherapy, occupational therapy, exercise physiology and more
- GP services, 715 health checks, and chronic disease care planning
- Child, youth and family support
- Disability assessments and support services
- Cultural and social support
- Family health and wellbeing programs (antenatal, postnatal, early childhood)
- End‑of‑life care
- Legal aid services
- Specialist medical services
Mob Link intake officers yarn with clients to understand what they need, then connect them with the right service quickly and safely.
Transport to Medical and Specialist Appointments
For Mob without access to transport, getting to health appointments can be a challenge. Mob Link helps remove this barrier by offering transport to:
- GP and primary care appointments
- Hospital visits
- Specialist appointments
- Regional medical services (when needed)
Transport can be arranged for individuals, families, and carers, with priority given to those who need extra support or don’t have access to other transport options.
Hospital and Home Visits
Health journeys don’t stop at the hospital door. Mob Link team members can visit people in hospital or at home to:
- Provide advocacy and support during care planning
- Assist with discharge and ongoing care needs
- Develop connections and assess what additional supports may be needed
- Coordinate follow‑up care across clinics and services
This ensures continuity of care with a culturally safe approach tailored to each person’s circumstances.
Social, Cultural and Community Support
Mob Link supports more than physical health – they also help mob stay grounded, connected and cared for. This includes:

Aged Care and Home Support Referrals
Elders and families can access:
- IUIH Home Support
- Aged care programs
- Regional Assessment Service (RAS) registration and assessment support

Chronic Disease Care Coordination
For people managing long‑term health conditions, Mob Link offers help with coordinating appointments, follow‑ups, planning, and specialist care. This support makes it easier to stay on top of ongoing health needs and improves long‑term wellbeing.

Mob Link is free and available to Aboriginal and Torres Strait Islander people of all ages – including families and carers. Services operate every day, with availability between 7:00am and 8:00pm.
