Disability Support Services – Direct Support

IUIH provides a range of core and capacity-building supports, including support coordination, plan management, allied health, assistance with daily living (including personal care and household tasks), social/community participation and transport.

Disability Support Services delivers direct support and care by for Aboriginal and Torres Strait Islander people. 

Our team of NDIS support workers provide services including: 

  • in-home support – meal preparation, cleaning and household chores, and personal care such as showering and dressing 
  • community access – assistance with shopping, transport, health appointments, attending social or community events.

  • In-home support services and community access supports can be provided for Mob living in the Moreton Bay / Brisbane North region. 
  • For Mob living in other regions, Disability Support Services provides connection to other service providers.

Who to refer 

Aboriginal and Torres Strait Islander people who have: 

  • a disability 
  • chronic illness, mental health, or circumstances the impact the ability to live independently in the community. 

How to refer

Email a referral form to [email protected]

  • Open Patient Record 
  • Create letter 
  • Create referral letter from template > Select from one of the following referrals: 
  • Disability Access and QCSS > Named person as “IUIH Referral – NDIS Access and QCSS.” 
  • Send to: IUIH Referral – NDIS Access and QCSS (IUIH NDIS)
