Disability Support Services – Support Coordination

IUIH provides a range of core and capacity-building supports, including support coordination, plan management, allied health, assistance with daily living (including personal care and household tasks), social/community participation and transport.

Disability Support Services provides support coordination to Aboriginal and Torres Strait Islander people.

Our team of support coordinators and social workers provide a range of services including:

  • support to understand plans and connections to the services and programs identified as a need
  • implementation of NDIS funding
  • advocacy – an unfunded program for Mob with an existing NDIS plan, but with no support coordination funding.
