Disability Support Services – Support Coordination
IUIH provides a range of core and capacity-building supports, including support coordination, plan management, allied health, assistance with daily living (including personal care and household tasks), social/community participation and transport.
General information
Disability Support Services provides support coordination to Aboriginal and Torres Strait Islander people.
Our team of support coordinators and social workers provide a range of services including:
- support to understand plans and connections to the services and programs identified as a need
- implementation of NDIS funding
- advocacy – an unfunded program for Mob with an existing NDIS plan, but with no support coordination funding.
Locations
Support Coordination services the South East Queensland region.
Referrals
Who to refer
Aboriginal and Torres Strait Islander people who have:
- a disability, and
- a NDIS plan, and
- support coordination funding in their NDIS plan, or
- no support coordination funding allocated, require assistance for support in engaging and managing services under their plan
