Understanding and using your NDIS plan can feel overwhelming, especially when you’re not sure where to start. Our support coordination team works alongside Aboriginal and Torres Strait Islander people and their families to help them understand their plan, access supports and make informed decisions about their care.
We take the time to understand your goals and connect you with services that best meet your needs. Whether you need help implementing your NDIS funding, finding service providers, coordinating supports or advocating for better outcomes, our team is here to help you get the most from your NDIS plan.

Who to refer?
Aboriginal and Torres Strait Islander people who have:
- a disability
- an NDIS plan
- support coordination funding in their NDIS plan
- no support coordination funding allocated, require assistance for support in engaging and managing services under their plan
How to refer
External
Email a referral form to our support coordination team.
IUIH Network (MMEx)
Patient record → Create letter → Create referral Letter from template → Disability Support Coordination. In the Named Person field, enter “IUIH Referral – NDIS Support Coordination”.
