The program pairs a dedicated family partnership worker and a nurse home visitor with the new mother, who will regularly visit the home from pregnancy, until the child is two years old.
The AFPP team provides first-time mothers with information, education, social, emotional and cultural support to build parenting skills and create a strong family.

Women birthing an Aboriginal or Torres Strait Islander baby, less than 28 weeks pregnant. Referrals may be considered over 28 weeks pregnant.
