For Mob connected to our clinics, your GP can refer you as part of your 715-health check.  For jarjum not yet at school, a referral isn’t required but we’ll need you to complete a separate consent form at the appointment.

If you’re not a client in our network, you will need to book a 715-health check with a GP at one of our clinics for a referral.
