GP Information

The YBB Family Wellbeing Service walks alongside families and communities to support them to be strong and resilient.

Our service provides culturally safe case management support for Aboriginal and Torres Strait Islander families.

Who to refer

  • Parents and caregivers and their kids who have multiple, complex needs (e.g. parenting, social and emotional wellbeing issues, housing, Centrelink, legal issues etc.) who would benefit from a culturally safe service
  • At least one member of the family who identifies as Aboriginal and/or Torres Strait Islander
  • Consent from the family to complete an FWS referral
  • Families who have a child/children under 18, including those unborn (if unborn, you must have the pregnant woman’s consent)
  • Residing in the bayside or Redlands region, including North Stradbroke Island and the Southern Moreton Bay Islands, Mt Cotton, Wynnum and Belmont.

MMEx:

  • Log into MMEx and click on emails “compose”
  • Add patient to the email
  • Find the Family Wellbeing Service Template in “letter templates” – *Referral – FWS YBB
  • Complete online referral form
  • Send referral to “YBB Referral – Family Wellbeing Service (YBB FWS)”
